HINT: If you have Adobe 5 ( or maybe another version) you can right click on the form and use it -- then you can save it. Adobe 9 does not let you save it.

NOTICE: After your vessel is approved as a Facility: Station Yankeetown needs updated Facility data sheets. A full sized sheet is available by clicking on the following:

Now that there is a new VESSEL FACILITY INSPECTION AND OFFER FOR USE FORM ( ANSC 7003 Rev 01-05) it is not necessary to include the Old Coxswain Assignment Sheet. IF you need more space just add a list. It is permissable to put ANY COXSWAIN and specify the Districe, Division and Flotilla on the 7003 form.

For Example: If you would allow ANY COXSWAIN in Div 15 to operate your vessel you would put 0701500. If you would allow ANY COXSWAIN in District 7 to operate your vessel you would put 0700000.

You should carry an ICS 201 from just in case you should be involved in an incident wherein you need to transer your duties to another vessel or team. The ICS 201 CG is a 4 page form for that purpose and may be obtained by clicking on the following link
